Fees and Services
To keep decisions about care between you and your doctor, this practice does not take insurance. Patients know ahead of time the cost of each service and pay directly at the time of service by debit or credit card. New Patient Evaluations: 2+ hours $375
Weekly therapy appointments: 50 minutes $105*
Other therapy appointments: 50 minutes $140

New Patient Evaluations
1-2 hours (60-120 minutes) | $375
This is a comprehensive initial evaluation. The psychiatrist will perform an assessment of your needs, condition or concern, and discuss your wellness goals. Typically, this appointment will last about one and 1/2 hours. This includes taking a detailed history about your symptoms, nutrition, and day-to-day activities. The doctor may ask many questions. However, please know you never have to go into more detail about a trauma than you feel ready to.
When indicated, a diagnosis is provided toward the end of the visit. Sometimes additional information is needed to establish a diagnosis. This may include a review of medical records from other doctors or behavioral health professionals, labs, imaging, etc. For this reason, please do not expect that psychotherapy or medication will necessarily be initiated at this visit. Requests to fill out forms for leave or other purposes will be deferred until the nature and severity of any psychiatric condition are determined.
Dr. Gilley will take the time to discuss your options with you. She will often incorporate counseling into these visits. Information about your condition or concern will be discussed so that you have a deeper foundation from which to make decisions about your care. You will be encouraged and have time to ask any questions you may have.
Psychotherapy
50 minutes | $140 per session

The number of therapy sessions needed varies widely among patients, depending on individual needs and patient goals. In Dr. Gilley’s experience, most patients do best and are more likely to experience lasting change when participating in weekly or every-other-week psychotherapy. This is especially true when starting treatment. When therapy is too spaced out, it can become less effective and may require the patient to remain in therapy longer to work towards the same benefits.
The type of therapy used depends on your needs and goals. Dr. Gilley uses psychodynamic, CBT, ACT, integrative and occasionally supportive therapy modalities. Some patients require only brief psychotherapy lasting on average twelve sessions. Others may participate long-term to address complex conditions, such as trauma, or ongoing concerns and major stressors. Treatment is tailored to individual patient needs and goals.
*The weekly therapy rate is available for patients attending 3 therapy appointments per 4 week period continuously until they wish to be seen less frequently or complete the therapy course. This rate may be used only for one treatment course per patient, and is not applied once therapy frequency decreases below the above requirements. This weekly rate applies to 50-minute therapy appointments only. It does not apply to medication services or other visit types.
30 Minute Follow-up Visits
30 minutes | $170
A follow-up visit may focus on the discussion of medication and/or considerations such as the impact of non-psychiatric medical conditions, nutrition, stress, activity and lifestyle changes on behavioral health. Counseling or brief psychotherapy interventions may be used; however, most Psychotherapy-only visits are scheduled for 50 minutes (see Psychotherapy-only visits, $130 per 50 minutes).
For patients interested in starting or changing medication, risks and benefits are discussed. The effects of medication on symptoms and side effects are followed. Labs may be ordered during these visits to assess for certain medical conditions, or to monitor the effects of specific medications. Frequency of medication follow-up visits is determined by type(s) of medication, illness severity, side effects, and other factors.
For safety reasons, controlled substances are not prescribed to telehealth patients at New Directions Psychiatry, PLLC. This includes medications like benzodiazepines, sedatives, hypnotics, opioids, (including maintenance medications like buprenorphine), certain sleep medications, and stimulants.
Combined Psychotherapy and Medication Follow-up Visits
50 minutes | $240
If you are taking medication and are being seen for psychotherapy, it is not necessary to focus on medications at every visit. However, if discussion of medication would involve more than a few minutes of check in, it is necessary to set aside time to for Dr. Gilley to evaluate effects, perform and review labs or vital signs if indicated, and make any needed changes to your medications. She will discuss with you how often medication-focused visits need to be, depending on type of medication, illness severity, side effects, and other factors.
For your convenience, medication visits can be combined with formal therapy visits, though you and Dr. Gilley may decide to do them separately, depending on the medication(s), complexity of the condition being treated, and issues being addressed in psychotherapy.
At combined visits, your medication, side effects, symptoms and response are discussed. Orders can be sent for labs, or changes or continuation of medication can be decided. The remainder of the hour would then be spent continuing your formal therapy.
Other clinical services may be offered at the fees and rates described in the practice’s current Fees and Services list, which is provided prior to the initial visit.

This telehealth-only, psychotherapy-oriented clinical practice does not offer Disability Evaluations or Fitness for Duty Evaluations (including Transportation/Aviation, or for carrying firearms), legal or Forensic Evaluations, or other assessments for legal cases (such as divorce or child custody/parental fitness). Professional ethics prevent psychiatrists from acting as both treating physician and as the evaluator of a patient for a third party such as an employer or for legal purposes.
